Fibromyalgia is a chronic medical condition in which there is long term body pain in the body parts which includes mainly muscles, joints, tendons, ligaments and tissues.
There is deep aching pain which ranges from mild to severe pain. There is not just chronic widespread pain but also a heightened pain response to pressure. Other symptoms include tiredness to a degree that normal activities are affected, sleep problems and troubles with memory.
It is most common in middle aged women; risk factor in males is less. People with rheumatoid arthritis are at increase risk. Sleep disorder, extreme fatigue and stiffness of joints are the most common feature of Fibromyalgia. It is commonly associated with psychic disorders such as anxiety, stress, depression etc.

Cause of Fibromyalgia
The exact cause of fibromyalgia is not known. It is often known to run in families but there is always a combination of genetic and environmental factors.
Stress is often an important precipitating factor in the development of fibromyalgia. It is frequently associated with stress-related disorders like chronic fatigue syndrome, posttraumatic stress disorder, irritable bowel syndrome and depression.
There is strong evidence that major depression is associated with fibromyalgia as with other chronic pain conditions.
Conditions associated with Fibromyalgia
- Rheumatoid arthritis
- Sleep disorder
- HIV infection
- Chronic fatigue syndrome
- Lyme disease
- Irritable bowel syndrome etc
Diagnosis of Fibromyalgia pain
Diagnosis entirely depends upon cause and symptoms. It is usually a clinical diagnosis made after ruling out other possible causes of pain. The American College of Rheumatology has the following diagnostic criteria:
- A history of widespread pain lasting more than three months – affecting all four quadrants of the body, i.e., both sides, and above and below the waist.
- Tender points – there are 18 designated possible tender points (although a person with the disorder may feel pain in other areas as well). Diagnosis is no longer based on the number of tender points.
Treatment of Fibromyalgia pain
There is no universally accepted treatment or cure for fibromyalgia, and treatment typically consists of symptom management. Integrated treatment plans that incorporate medication, patient education, aerobic exercise and cognitive behavioral therapy have been shown to be effective in alleviating pain and other fibromyalgia related symptoms.
A small number of people benefit from the SNRIs duloxetine and milnacipran and the tricyclic antidepressants (TCAs), such as amitriptyline. The anti-convulsant drugs gabapentin and pregabalin are also used to reduce pain.
